Reference forms:
You will need three reference forms filled out prior to acceptance into the program.
You can have those filling out your reference forms submit those by clicking here.

Housing:
You will enjoy the privilege of having roommates for this course. You will need to bring your own bedding, wash cloth, towel, and personal toiletry items. Families will have a slightly higher cost due to housing and meal needs.

Guests:
If you are expecting guests during the course, please speak to our Administrator so that arrangements can be made for housing/food. It is only by special arrangement/emergencies that any LIFE student may miss classes during the term. If the student is wanting to travel on the weekends and be off of Campus, he/she must submit that request in writing. You will find the form for this in the “Forms” section.

Laundry Facilities:
Laundry facilities are available on site.

Meals:
The cost per meal for guests is $7.00/meal. Payable to “LIFE.” All visitors should contact LIFE prior to arrival so that those preparing the meals can be prepared. The number to contact is 1-888-MAT 28:19.

Dress:
(The final determination as to the appropriateness of clothing will be left to the discretion of the on-site staff, namely Pastors Scott and Phillip, and Seth Dixon)

For Men:
Classroom attire should be suited for learning. No shorts or t-shirts with off-color designs/slogans. It should be kept in mind that the uniform of a Christian shows which side he/she is on. Recreational attire should be modest with no tight fitting clothing. Swimming attire–shorts should come close to the knee, a colored t-shirt must be worn.

For Ladies:
Classroom attire should be suited for learning. Skirts should come below the knee. No cleavage should be shown. No tight fitting clothing. Recreational attire should be modest with no tight fitting clothing. Swimming attire–shorts should come close to the knee, a colored t-shirt with appropriate under clothing must be worn.

Adornment:
The Seventh-Day Adventist Church Manual Speaks… “To dress plainly, abstaining from display of jewelry and ornaments of every kind, is in keeping with our faith.”—Testimonies, vol. 3, p. 366. It is clearly taught in the Scriptures that the wearing of jewelry is contrary to the will of God. ‘. . . not with broided hair, or gold, or pearls, or costly array’ is the admonition of the apostle Paul (1 Tim. 2:9). The wearing of ornaments of jewelry is a bid for attention which is not in keeping with Christian self-forgetfulness. SDA Church manual p. 176, Paragraph 4

Attendance:
Class attendance is mandatory. If you have visiting family or friends, it is your duty to let them know that you will not be able to miss any classes while they are visiting. Any absence must be approved by Seth Dixon, Phillip Sizemore, or Scott Moore. Excess absences for any reason will be grounds for dismissal.

Outreach:
Mandatory. One of LIFE’s strengths is the opportunity to use what you learn. Every student in all of LIFE’s courses will be required to participate in Outreach activities. Refusal to participate or skipping Outreach activities will not give you the opportunity to experience soul-winning first hand and will result in your dismissal from the program.

Duties while at LIFE:
During your time here at LIFE, you will be assigned specific weekly projects that aid in the upkeep of the facilities, as well as the preparation of the meals. The Bible says that the one who is faithful in the least will be faithful in much. Your participation in the activities assigned to you on a weekly basis are appreciated, necessary, and mandatory. Failure to participate in the assigned weekly duties will be grounds for dismissal.

Dismissal of student/Students desire to leave the program:
LIFE reserves the prerogative to dismiss any student for their willful refusal to participate in the activities of LIFE.

If you are dismissed for willful refusal to follow LIFE’s policies as listed in this handbook or those policies shared with you throughout the LIFE course, there will be no refund of monies paid.

If a student wishes to leave the program of their own choice within the first 15 days of the program, that student will receive a 50% refund of monies paid.

If a student wishes to leave the program of their own choice after the first 15 days but before the 30th day of the program, that student will receive a 25% refund of monies paid.

If a student wishes to leave the program of their own choice after the first 30 days of the program, that student will not receive any refund of monies paid.

Weekend Leave Policy:
You must have administration approval for any leave during your stay at LIFE. If you are under the age of 18 you may not leave campus without a faxed permission slip from your parents. This fax must include their signature and the location to which you will be going and the method of transportation. Fax number: 352-557-4554. Student must return by 12:00 pm Sunday afternoon. There will be no Weekend Leaves approved during the Evangelistic Meeting. All students expecting to leave during the Thursday/Friday of Thanksgiving must fill out a Weekend Leave request form and must return by 12:00 pm the Sunday following Thanksgiving.

Spring/Fall Four Week LIFE on the EDGE/Cycle of Evangelism/Communications Class–$1500

Insurance:
LIFE does not provide health insurance for students. Each student is responsible for his/her own health insurance.
